Replacing a Urostomy PouchStep 1slide 1 of 4 slide 1 of 4, Step 1, Remove your old pouch by peeling away the barrier and gently lifting the pouch while pressing down on the skin below the pouch. Dispose of the old pouch. Step 2slide 2 of 4 slide 2 of 4, Step 2, Clean your skin with a wet washcloth or wipe. Measure your stoma, if you need to. Step 3slide 3 of 4 slide 3 of 4, Step 3, Cut an opening in the new barrier slightly larger than the stoma. Remove the paper backing from the barrier so that the adhesive is exposed.
