Sodium RecordUse
a form to record the
sodium content of the foods you eat or drink each day.
This record will help you see whether you are getting the prescribed daily
amount of sodium in your diet. Make a chart with 3 columns and as many rows you need for meals and snacks. In the first column, record your meals. In the second column, record what you eat or and drink, and in the third, record the total number of milligrams or grams of sodium in the meal. Take this record with you when you
visit your health professional. See the example below. My doctor recommends that I have
